Background Ethical climate refers to the shared perception of ethical norms and sets the scope for what is ethical and acceptable behaviour within teams. Aim This paper sought to explore perceptions of ethical climate amongst healthcare workers as measured by the Ethical Climate Questionnaire (ECQ), the Hospital Ethical Climate Survey (HECS) and the Ethics Environment Questionnaire (EEQ). Methods A systematic review and meta-analysis was utilised. PSYCINFO, CINAHL, WEB OF SCIENCE, MEDLINE and EMBASE were searched, and papers were included if they (...) sampled healthcare workers and used the ECQ, HECS or EEQ. Ethical consideration Ethical approval was not required. Results The search returned 1020 results. After screening, 61 papers were included ( n = 43 HECS, n = 15 ECQ, n = 3 EEQ). The overall sample size was over 17,000. The pooled mean score for the HECS was 3.60. Mean scores of individual studies ranged from 2.97 to 4.5. For the HECS studies, meta-regression was carried out. No relationship was found between the country of the studies, the study setting (ICU v non-ICU settings) or the mean years of experience that the sample had. For the ECQ, sub-scales had mean scores ranging from 3.41 (instrumental) to 4.34 (law) and were all observed to have significant and substantial heterogeneity. Three studies utilised the EEQ so further analysis was not carried out. Conclusions The above results provide insight into the variability of scores as measured by the HECS, ECQ and EEQ. To some extent, this variability is not surprising with studies carried out across 21 countries and in a range of healthcare systems. Results also suggest that it may be that more local and context specific factors are more important when it comes to predicting ethical climate. (shrink)

Tuberculosis (TB) is one of the world’s leading causes of death due to infection and efforts to control TB would be substantially aided by the availability of an improved TB vaccine. There are currently nine new TB vaccines in clinical development, and the first efficacy trials are due to commence in 2009. There are many complex ethical issues which arise at all stages of TB vaccine development, from the need to conduct trials in developing countries to informed consent and the (...) process of ethical review. While it is important that these issues are discussed, it may also be timely to consider the challenges which may arise if a vaccine in clinical development proves to be highly effective. We examine a number of scenarios where decisions on the deployment of a new TB vaccine may impact on the rights and liberty of the individual. (shrink)

Harriet Taylor Mill and Anna Wheeler are two nineteenth-century British feminists generally over-shadowed by the fame of the men with whom they co-authored. Yet both made important and interesting contributions to political thought, particularly regarding deconstruction of (i) the patriarchal institution of marriage; and (ii) the current property regime which, in dominating workers, unfairly distributing the product of labour, and encouraging ‘individualism’, they believed did little to maximize the general happiness. Both were feminists, utilitarians, and socialists. How they link these (...) elements is both interestingly similar, and interestingly different. This article has four aims. Firstly, to make a strong claim concerning their authorial hand in works often considered to be solely the work of their male co-author. Secondly, to sketch those co-authoring relationships, and consider whether Taylor and Mill may even have consciously constructed their early letters ‘On Marriage' based upon what they knew of Thompson and Wheeler’s relationship. Thirdly, to map out their shared (though not identical) claim that marriage was a form of slavery, and the proposals they offered to free women from the domination of patriarchal relationships. The extraordinary career of the British Quaker polymath, Silvanus Phillips Thompson (1851–1916), encompassed fame in physics, electrical engineering, mathematics, history of science, educational method, painting, music, textbooks, X-rays, popular lectures, the promotion of women's rights, book-collecting, and not least his leadership in encouraging fellow Quakers to embrace the challenging results of research in the natural sciences. His public-facing career, with a reputation that ranged across Western Europe at least, centred on the sincere yet critical communication of new technical and (...) historical knowledge, in a mastery of four languages. Yet his kaleidoscopic work has not received any sustained historical examination since the Life and Letters produced by his widow Jane and daughter Helen in 1920. The centenary of his death was marked by an interdisciplinary workshop at the Westminster (Quaker) Meeting-House, “‘A Many-sided Crystal’: The Quaker Physicist and Electrical Engineer Silvanus Phillips Thompson” on September 16, 2016. This spotlight section of Centaurus captures four of the revised contributions to that event, and these cover Thompson's contributions to historical theory, biographical practice, and commercial technology, as just a few elements of the rich and complex legacy that emerged posthumously from his multifarious, polymathic talents. These collectively point us to a revised view of Thompson as a pre-First World War European figure who gained his authority not from specialization in a single area of esoteric research, but from a life of public service that integrated the literary arts and historical writing with sciences and engineering, all incorporated within an active Quaker practice. The papers in this collection thus show how Thompson came to be an historian of science with an unprecedented mastery of contemporaneous techno-scientific arts and sophisticated skills in historical-biographical writing, working harmoniously in a secular world with a rigorous yet non-dogmatic faith. Was Silvanus Phillips Thompson primarily a physicist, electrical engineer, biographer, or teacher? His obituarists could not agree. I argue Thompson was in fact a polymathic generalist who, as a philanthropic Quaker, worked not to promote his own expertise but rather to ensure the public was swiftly informed of the most important techno-scientific research and applications of his contemporaries. I illustrate this in a comparison of Thompson and his longer-lived friend Oliver Lodge: working in closely-related areas, they had (...) contrasting profiles and commitments. After inspecting Thompson's work as textbook author and bibliophile, I resolve his apparently paradoxical status as both radical critic and figurehead of multiple institutions. Finally, to flesh out his friends' representation of him as a “many-sided crystal,” I analyse Thompson's multi-facetted posthumous reputation, especially in reviews of the Life and Letters written by his widow Jane and daughter Helen exactly 100 years ago. This symposium provides five case studies of the ways that John Dewey's philosophy and practice were influenced by women or "weirdoes" (our choices include F. M. Alexander, Albert Barnes, Helen Bradford Thompson, Elsie Ripley Clapp, and Jane Addams) and presents some conclusions about the value of dialoging across difference for philosophers and other scholars.

The traditional military-territorial model of the nation state defines international duties in terms of protecting citizens' property from foreign threats. In this 1992 book about the principles of the US agricultural policy and foreign aid, Professor Thompson replaces this model with the notion of the trading state that sees its role in terms of the establishment of international institutions that stabilize and facilitate cultural and intellectual, as well as commercial, exchanges between nations. The argument focuses on protectionist challenges to (...) foreign aid and development assistance programs, and engages with the views of a variety of economists, commodity organizations, and philosophers on world hunger and development. This article uses philosopher Miranda Fricker’s work on epistemic injustice to shed light on the legal concept of the fiduciary, alongside demonstrating the wider contribution Fricker’s work can make to business ethics. Fiduciary, from the Latin fīdūcia, meaning “trust,” plays a fundamental role in all financial and business organisations: it acts as a moral safeguard of the relationship between trustee and beneficiary. The article focuses on the ethics of the fiduciary, but from a unique historical perspective, referring back to the (...) original formulation of the fiduciary within a familial context to investigate presuppositions regarding agential capabilities, whilst also paying attention to the power mechanism embedded in the trustee–beneficiary relationship. Using Fricker’s theory of pre-emptive testimonial injustice, the analysis elucidates the impact of cumulative beneficiary silencing in contemporary contexts, and the article uncovers ethical issues of an epistemological kind at the core of the fiduciary—of epistemic injustice. Social justice has strong historical roots in public health. This does not mean that we always understand what it entails when conducting an ethical analysis of a particular public health program. This article shows that Powers and Faden’s theory of social justice can provide important insights and nuance to such an analysis. The Ontario human papilloma virus vaccination program that is underway in Canada provides an important and timely case where we can surface ethical issues pertaining to social justice that (...) may otherwise remain unarticulated in the context of this program. This analysis focuses on the normative issues raised by the prioritization of a school-based program for girls only. It also examines the relevant domains of well-being identified in Powers and Faden’s theory to see whether the program is likely to enhance the well-being of those for whom it is most important. In this article, I discuss the structural domination of whiteness as it intersects with the potential of individual critique and reflexivity. I reflect on my positioning as a white nurse researcher while researching international nurse migration. I draw on two large qualitative studies and one small focus group study to discuss my reactions as a white researcher to evidence of institutional racism in the British health services and my growing awareness of how racism is reproduced in the British nursing profession.

The aim of this literature review was to compose a narrative review supported by a systematic approach to critically identify and examine concerns about accountability and the allocation of responsibility and legal liability as applied to the clinician and the technologist as applied the use of opaque AI-powered systems in clinical decision making. This review questions if it is permissible for a clinician to use an opaque AI system in clinical decision making and if a patient was harmed as a (...) result of using a clinician using an AIS’s suggestion, how would responsibility and legal liability be allocated? Literature was systematically searched, retrieved, and reviewed from nine databases, which also included items from three clinical professional regulators, as well as relevant grey literature from governmental and non-governmental organisations. This literature was subjected to inclusion/exclusion criteria; those items found relevant to this review underwent data extraction. This review found that there are multiple concerns about opacity, accountability, responsibility and liability when considering the stakeholders of technologists and clinicians in the creation and use of AIS in clinical decision making. Accountability is challenged when the AIS used is opaque, and allocation of responsibility is somewhat unclear. Legal analysis would help stakeholders to understand their obligations and prepare should an undesirable scenario of patient harm eventuate when AIS were used. (shrink)

Early twentieth-century Italian poetry experiences a crisis in confidence concerning the expressibility of rhythm. Dino Campana's writings exemplify the processes the poet goes through in order to write rhythm. Rhythm is difficult to deal with because it is both sacred and tired. These two incarnations of rhythm lead Campana to different modes of expression; from more traditional definitions through to more fluid definitions. Two strands of analysis reveal themselves as central to understanding Campana's theoretical stance, namely fluidity and movement. This article presents a critique of Philip Pettit’s concept of ‘freedom as non-domination’ and provides an alternative theory of both domination and republican political freedom. I argue that Pettit’s neo-republican concept of domination is insufficient to confront modern forms of domination and that this hampers his concept of republican freedom and its political relevance under the conditions of modernity. Whereas the neo-republican account of domination is defined by ‘arbitrary interference’, modern forms of domination, I argue, are characterized by routinization and (...) systemic forms of control and subordination. In the end, the neo-republican account of domination is more appropriate for 17th- and 18th-century social institutions rather than those that persist under modernity. In its place, I propose a more dynamic concept of domination and rework the concept of freedom in order to place the republican tradition within the context of modernity itself. I argue that the core insight of republicanism is its emphasis on the arrangement and reformation of social institutions to enhance social freedom. From this, I argue that republicanism can take its place as a more attractive alternative to liberal theory. (shrink)

We consider various curious features of general relativity, and relativistic field theory, in two spacetime dimensions. In particular, we discuss: the vanishing of the Einstein tensor; the failure of an initial-value formulation for vacuum spacetimes; the status of singularity theorems; the non-existence of a Newtonian limit; the status of the cosmological constant; and the character of matter fields, including perfect fluids and electromagnetic fields. We conclude with a discussion of what constrains our understanding of physics in different dimensions.
